Action Cares

Meets the housing and supportive services needs of medically fragile individuals and their families.

 

Action Cares is Action Ministries’ federally funded housing program designed to bring about family stability by meeting the housing and supportive services needs of low-income people who are living with HIV/AIDS and their families. Through Action Cares, Action Ministries’ team of housing professionals work with each and every member of the family unit to first identify safe and sanitary housing options that meet industry standards for habitability, connect with the appropriate short-term rental assistance, giving a sense of security for all household members and design family plans with realistic goals, establishing the path to long-term success. Once on the Action Cares client roster, families are never alone as they are paired with the appropriate case manager who accompanies them on this collaborative journey to family success.

Action Cares ends homelessness by:

  • Providing short-term assistance with rent, mortgage and utility payments (including deposits) for individuals with HIV/AIDS who are at imminent risk of becoming homeless or homeless.
  • Ensuring that clients maintain their housing for at least a year.

  • Helping clients build enough income to maintain their housing after the program.

  • Assisting clients in accessing community resources to help address their needs.

To be eligible for Action Cares, you must meet the following requirements:

  1. You must be diagnosed with HIV/AIDS and have documentation showing your HIV/AIDS status from a doctor.
  2. You must meet federal income guidelines.

HOPWA only accepts applications with an agency referral form from the Ryan White Clinic.
